Keeping secondary students responsible for their actions is something I feel really strongly about! One way I do this is by keeping responsibility reports on my desk at all times. Responsibility reports look like this:
When I collect homework from my students I expect every student to turn something in. Students who did not complete their homework pick up one of these forms from my desk, fill it out, and turn it in when the rest of the class turns in homework. I also have students fill out a form for classwork they miss. I keep all of the forms in a binder that is separated with dividers for each class.
