How Do You Keep Track of Students' Reading at Home?

 

Our school has a requirement that every child reads at home a certain number of minutes each evening based on grade level.  I needed a way to keep track without getting buried in paperwork.  I created the form below. 

I send a copy home each Monday stapled to the math packet for the week.  Everything is due Friday, so I am checking things off once a week.
