Every year in our district, we have our open house before school starts. This is when the teachers and students meet and greet, but it is also a day of paperwork. We often provide parents with lots of information about our school and classrooms. We also have them update their information for our system, sign our technology use forms, and much more.
One of the forms that I like to add into this set is my own personal student information form that I collect from each student and place in a binder.
On one form I have at my fingertips virtually everything I need - information about contacting parents (including the preferred way to contact them), religious restrictions, medical concerns, allergies, siblings, academic information, and much more.
